The mod maker messed up the naming. He has: lthead_hockey_v5.dds and it should be:
lthead_hockey_v05.dds
So find the texture file (lthead_hockey_v5.dds); should be in the game's texture folder after installing (Data>Texture); and rename it by adding the "0" (zero) between the v and 4
That should do it for you.
